Title 33, Chapter 21, Section 15
( 33-21-15)
(a) Every health maintenance organization shall annually, on or
before March 1, file with the Commissioner of Insurance, on forms to
be designated by him and certified by at least two principal
officers of said health maintenance organization, an annual
statement as of December 31 of the preceding year and a copy of said
report shall also be delivered to the commissioner of human
resources. (b) Such report shall be on forms prescribed by the Commissioner of
Insurance and shall include: (1) A financial statement of the organization, including its
balance sheet and receipts and disbursements for the preceding
year certified by an independent public accountant; (2) Any material changes in the information submitted pursuant to subsection (b) of Code Section 33-21-2; (3) The number of persons enrolled during the year, the number of
enrollees as of the end of the year, and the number of enrollments
terminated during the year; (4) A summary of information compiled pursuant to paragraph (3) of subsection (b) of Code Section 33-21-3 in such form as required by the commissioner of human resources; and (5) Any other information relating to the financial condition or
performance of the health maintenance organization as is necessary
to enable the Commissioner of Insurance and the commissioner of
human resources to carry out their duties under this chapter. |
