(1) "Health care provider" means any hospital or ambulatory
surgical or obstetrical facility having a license or permit issued
by the department under Article 1 of this chapter.

to provide or otherwise lawfully providing health care services,

(6) "Health care provider" means a physician, chiropractor,
podiatrist, optometrist, pharmacist, or physical therapist who is
licensed or otherwise regulated under this title.
