(i) "Enrollee" means an individual who has elected to contract
for or participate in a health benefit plan for that
individual or for that individual and that individual's
eligible dependents and includes that enrollee's eligible
dependents.
(2.1) "Enrollee" means an individual who has elected to contract
for or participate in a managed care plan for that individual or
for that individual and that individual's eligible dependents.
(2) "Enrollee" means an individual who has elected to contract for
or participate in a health benefits plan for that individual or
for that individual and that individual's eligible dependents.
"Enrollee" means any person entitled to receive health care
services or reimbursement for such services pursuant to a contract
with a health maintenance organization, whether the contract is
with the person entitled to receive those services or
reimbursement [...]
(4) "Enrollee" means the individual who has elected to contract
for or participate in a health benefit plan for himself or himself
and his eligible dependents.
